Iron Deficiency That Never Improves Is Telling You Something

Low iron since childhood is not a personality trait. If supplements are not correcting it, something else is going on.
"My iron has been low since I was a child. I do not remember it ever being normal."
I hear this constantly, usually mentioned casually at the end of a consultation, as though it were a fixed feature of the person rather than a problem with a cause.
Iron deficiency that does not correct with iron supplements, and does not correct with the dietary changes you have made, is not normal. It means one of two things: you are losing iron, or you are not absorbing it.
Ferritin is the number that matters
A standard blood count measures haemoglobin and haematocrit. Those can look perfectly acceptable while your iron stores are empty.
Ferritin measures the stores. This is the marker that gets missed, and it is the one that explains why a woman with a normal blood count still feels exhausted, loses hair and cannot recover from exercise.
Ask for ferritin specifically. If it is not on your report, it was not tested.
Reason one: you are losing it
The most common cause in women is menstrual blood loss. If your periods are heavy, and particularly if they are heavy enough to interfere with your life, that is a plausible explanation and it deserves proper investigation rather than a shrug.
Heavy periods are extremely common. Common is not the same as normal.
Other sources of loss sit in the digestive tract and need medical assessment. This is not something to guess at.
Reason two: you are not absorbing it
This is the one nutrition can address directly.
Gut health. If the gut lining is inflamed, absorption suffers regardless of what you swallow.
Stomach acid. Adequate stomach acid is required to liberate iron from food. Long term acid suppressing medication changes this picture.
What you take it with. Iron absorption improves substantially with vitamin C. It is reduced by tea, coffee, calcium and some fibre supplements taken at the same time.
The form you are taking. Some iron formulations are considerably better tolerated and better absorbed than others. A supplement that causes so much constipation that you stop taking it is not working, whatever the label says.
The type of iron in your food. Heme iron, from red meat, poultry and fish, is far more readily absorbed than the non heme iron found in plants. This is one of the genuine nutritional arguments for including fresh red meat.
What to do about it
- Test ferritin, not only haemoglobin
- Investigate heavy periods rather than accepting them
- Take iron with vitamin C, away from coffee, tea and calcium
- Address gut health, because absorption depends on it
- Retest after a defined period, so you know whether what you are doing is working
Why it matters beyond tiredness
Low iron affects energy production at the cellular level. It affects hair, recovery, mood, thyroid function and cognitive performance. Women often attribute all of that to age, or stress, or hormones, when a single unaddressed deficiency is contributing to all of it.
You cannot out train a deficiency, and you cannot out supplement one you are not absorbing.
